Participating providers are independently owned and operated practitioners or practices. They are not employees, agents, partners, or representatives of The Salt Lake Collective solely by virtue of their participation in the Collective.
Each provider is independently responsible for their professional services, clinical decisions, licensing and credentials, scheduling, fees and billing, insurance participation, medical or clinical records, informed consent, privacy practices, and compliance with applicable professional and legal requirements.
Inclusion in The Salt Lake Collective should not be understood as a guarantee or endorsement of a particular provider or as creating a provider-patient or provider-client relationship with The Salt Lake Collective.
